I'd always be honest on all forms. I've had situations where to answer questions from the other parties doctor. He tried to guide the responses to make me seem worse than I was so he could report that I exaggerated the replies on my forms.

Oh Jay <<<<hugs>>>> I hate seeing people having to go through the all the stress of claiming PIP. Never, ever, ever give them a good day scenario.
Like Hugh's wife I too was once a decision maker, well before PIP, and with certain benefits I just knew some people were downplaying their condition but had to make a judgement on what I had before me and on many occaisions I underlined the option to appeal in pencil in the letter that I had to send out.
One of the main reasons I transfered over to being a DM on serious fraud cases.

As long as you've got evidence to back up what you put on the forms then they can't dispute anything. Yes you put everything down as if it's your worse day, that's a given, even though you're still being honest.
You have to look and try to answer every question on the form because they are 'loaded questions' designed to make you fail if you answer them all as the form expects you to. This is one of the main reasons why people fail, that and not supplying enough evidence. The size of the boxes you write your response in are small because they want to you write yes/no or 1 sentence to fit the boxes. You will fail if you comply to that. 1 question should be half a page or more, more the better so ignore the space, just put continued on page xx. etc etc etc.

Evidence is an issue in as much as it is all on record but not in my hands. You can see my issues with your own eyes and I can provide far to many ESA assessments finding me unfit for work on the first visit plus my anti anxiety meds but there is nothing I can provide.
As someone who also suffers clinical depression I have also kept myself isolated. It is only now my health has declined I have no option but to seek 3rd party help. It will be months before anything comes of this so I am trying not to get anxious about it
My mental condition stems from having Encephalitis when I was 6 years old which basically left me susceptible to these conditions and I guess what they would call PTSD now from being physically abused by my mother. So it has been a life long battle. My earlier issues on this forum with how I interacted with a couple of members is also part of it

With my GP practice, I order my prescriptions online using 'SystemOnline' also with this I can look at all my notes from my GP and consultants. Using this I can print out all I need and use as evidence, this really helped my case.
From what I can gather is whatever you put on the forms, if it isn't backed up with evidence it gets ignored. So if you put you have anxiety, if you GP doesn't know this or if it's not 'diagnosed' then you don't have it in their eyes.
If you don't have access, then you can pay to get all your medical notes printed out and use that if it will help.
As with depression, you should be diagnosed and given a proper diagnosis, with a code number. For I have F33.2 and diagnosed. Too many people state they have depression and anxiety but never seen a GP about it let alone a mental health professional, then wonder why what they've said doesn't get taken into account.

I was first diagnosed with both conditions way back in the early 90s but this was just a Doctors diagnosis. I have had counselling and CBT but tbh my Doctors over the years have been pretty useless in getting me any real diagnosis ie through a Psychiatrist. The last time they tried to "help" I had to call a number then get triaged over the phone which I find unbelievable and ended up in CBT which helped a little but then the pandemic hit and all the good it did quickly vanished. I guess from what you say there then I am pretty much stuffed as far as points for mental health so I guess I will have to hope my physical health will be enough

Judging by Jeremy Hunt's budget, it looks like those with mental health issues and bad backs will be helped (forced) back into looking for work and the usual penalties for those that can't (or won't in their eyes). It's not looking good either ways.
